This is a Word file containing the Division III versions of the NCAA Student-Athlete Statement (Form 08-3c), NCAA Drug-Testing Consent Form (Form 08-3f), and NCAA Drug-Testing Consent Form for the Division III Year-Round Pilot (Form 08-3f-1) in one-page, legal-size formats that can be printed and copied back-to-back to create a single-sheet form that can be used to administer these forms to student-athletes. This study presents a comprehensive profile of Division III member institutions and conferences using extensive data from a variety of sources. Categories include demographics; sport sponsorhsip; admission selectivity and test scores; tuition and financial aid awards; recruiting, operating, and total program expense; and team championship selection and performance. Extensive cross tabulations and summary data is included as well as an executive summary.

This is the final report of an NCAA Initiative Grant funded study that involved players, coaches, fans, officials and campus administrators in the following:

- Review of literature and research in the area of sportsmanship in intercollegiate athletics.

- Completion of an attitudinal survey among key conference stakeholders as a benchmark for campus conversations and to measure changes in future years.

- Facilitated “conversations on character in sport” on each WIAC member campus to discuss the definition of positive sports behavior, the relative importance of the topic, accountability for behaviors in each group, and specific actions that might influence attitudes and behavior in the long term.

- Identification of realistic strategies as part of an action plan for implementation throughout the conference in future years.
